How do you unlock this medal?
Note: I am NOT asking how do you achieve the medal. This medal is not in my list and I would like to work on achieving it.

You can achieve it even if it doesn't show. It turns up every time you're QFT'd, if memory serves.

yeah, i've never seen this medal on my list, even when QFT'd, but i have not heard of any rank requirement, so ...
