I have added my Throne of the Revered 3 months ago according to my planet (Colony Age: 3months) but for the past 4 days (yes., 4!) when I try to upgrade my Throne I get this message "
You must wait 3 months from the last upgrade before activating this again."

I, of course, upgraded it to mark II the same day I added it so it should be at 3 months. I know these medals are usually off by a day or two because of rounding up or down but 4 days late makes little sense. Has anyone else run into this issue ?

You already know you just have to wait. What's the point in making a thread about it? If it's really that much of an issue, send in a support ticket.

It's called Dan maths, where 6 months is 6 x 31 Days

Yep, this, except I probably wouldn't blame Dan too much for it because it is a little confusing to program otherwise.
"Month" is a bit vague, to say the least.
Can refer to 4 weeks / 28 days, the time between the same date over consecutive months, 365.2422 /12 or 29,30 or 31 days.
*shrugs* That's a fair bit of built-in variation.

I do get the frustration of waiting, but suspect it's just the one displayed (which we go on) isn't quite the same as the "clock" which is ticking down.
I try not to worry too much about it and treat it only as a rough guide.

Mhhhm, yes and no. You can't blame him for months being variant, but you can blame him for not just picking something easier, say... Increments of weeks.

I will admit that seeing "You must wait another 16 weeks before using this again" may be a little disheartening to read (as opposed to 4 months), but at least it's accurate.
